TERMS AND CONDITIONS
Professional Microfilm, Inc. (hereinafter “PMI”), in consideration of the prepaid annual service charge, agrees to perform maintenance service on the equipment listed by model and serial number according to the following terms and conditions:
PMI will provide emergency service to calls as reasonably requested by the customer at no additional charge for labor. Said service is to be performed during normal working hours (8:00 AM to 5:00 PM) not including Saturdays, Sundays and Holidays.
Customer agrees to notify PMI in writing whenever the geographical location of equipment is changed.
There will be no additional charge for normal replacement parts. This agreement does not include changes of toner, developer fluids, paper, PC boards, glass parts, supplies, drum, lamps, installation of accessories, or major overhauls. Abuse or damage to the equipment due to repairs by someone other than the authorized PMI representative is not covered by this agreement. If equipment is altered by Customer or Customer utilizes a supply item, which in the judgment of PMI, increases the cost of maintenance to be performed under this agreement, PMI will propose either an incremental charge or request Customer to discontinue said conduct.
This agreement becomes effective on the date above specified, after PMI has received payment of the yearly charge. This agreement shall remain in force for one year and will be renewed automatically unless either party notifies the other in writing of cancellation at least 30 days prior to the contract year end. Customer agrees to pay then the current annual rate at the beginning of each contract year.
This document embodies the entire agreement between PMI and the Customer as to the subject mater hereof and cannot be modified or rescinded without a written document to such effect signed by both parties.
